- Our Case Managers identify what your needs are, as expressed by you and your circle of support.
- Our Case Managers facilitate referral to appropriate service providers.
- Our Case Managers help to remedy a wide array of problems and barriers: medical, mental health, vocational and housing issues.
- You will receive their assistance in becoming empowered to increase your functioning in the community.
